    Practice Policy and Misses Practice Form

    Athletes who miss practice must complete the Missed Practice Form. Consequences for failing to report missed practices or to scan the attendance codes at the start of practice:  3 Missed: Meeting with Athlete and Coach, 5 Missed: Meeting with Athlete, Coach and Parent,  6 Missed:  Dismissal from the team
